An employee was depalletizing and moving a 7-foot tall, 1,500-pound power distribution cabinet for installation. The power distribution cabinet became unsteady and fell onto the employee, causing injury to their vertebrae that required hospitalization.

While stripping a 12 AWG (American wire gauge) conductor, he was shocked, causing him to fall and hit his head on the concrete floor. The injured employee was performing routine maintenance on a fan belt/pulley. The injured employee and another person were moving the pulley back and forth manually to find a bearing issue. The injured employee's left ring finger got caught between the belt and the pulley wheel, resulting in a soft tissue amputation.

As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
