An employee was greasing a pumping unit. He was standing on the T-block when the weights swung. He moved back but was caught between the weights and the arm of the unit. He tried to catch himself, and his right hand was pulled into the pulley, resulting in amputation of his right little, ring, middle, and index fingers.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.
