An employee was operating a table saw to cut pieces of lumber measuring 2x2x36 inches. The blade contacted the employee's left thumb, cutting half of the distal phalange bone and amputating the thumb tip along the bottom of the fingernail.

Mayco Manufacturing, LLC

An employee was pouring molten lead into molds. When lead solidifies, the lead is removed from the mold using an eye bolt and chain attached to a bolt that is poured into the lead cylinder. The employee attempted to lift the lead cylinder from the mold, but the mold was stuck to the lead. The employee attempted to clean the surface of the floor underneath the mold using his hand while the mold and lead cylinder were suspended in the air. The mold released from the lead cylinder and struck him in the right thumb, amputating the thumb behind the thumb nail.

An employee was helping unload a skid steer off a truck skip. While positioned with his left foot on the skid steer's step and his right foot on the tire, he was helping unhook the chains from the truck skip and slipped and fell backward, landing on the rear rail of the truck skip. He suffered six broken ribs and a flail chest injury.

While walking down a set of stairs that had four steps, an employee missed a step and slid down the stairs, catching his leg between the upper and lower hand rail. The employee's knee twisted, tearing a thigh muscle.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.
