Final narrative
A maintenance employee was troubleshooting a conveyor while standing within the perimeter guarding system (fencing). His right ring finger became caught between the chain and sprocket on the conveyor resulting in amputation of the fingertip. The chain and sprocket were not guarded at the time of the incident.
On 12/7/2020, an employee was pushing a die out of a hydraulic press when their fingers were smashed between the die they were pushing and a stationary plate on the press frame, resulting in a partial amputation of two fingertips. The machine was locked out at the time of the incident.
