His right ring finger became caught between the chain and sprocket on the conveyor resulting in amputation of the fingertip. The chain and sprocket were not guarded at the time of the incident.

On 12/7/2020, an employee was pushing a die out of a hydraulic press when their fingers were smashed between the die they were pushing and a stationary plate on the press frame, resulting in a partial amputation of two fingertips. The machine was locked out at the time of the incident.

An employee was using a table saw to cut strips of plywood. The employee's left hand came into contact with the sawblade, which amputated the index and middle fingers as well as lacerating the hand. The employee was hospitalized, requiring surgery.

As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
