An employee was using a wire spooling machine when the machine malfunctioned and would not start. The employee was working to manually start the machine when it moved. The employee sustained an amputation to their right middle fingertip, degloving to the right little finger, and a fractured right ring finger.
An employee was operating a slitter machine with a wrap roll of steel mesh. The employee's gloved left hand became caught in the wrap roll, which pulled his hand into the roll and lifted his body off the floor, resulting in hospitalization for two broken bones in the left forearm.
