Employee 2 was working as ground support and repositioning a hoist that was being used for a lighting fixture. During this move the lift started to tip over. Employee 2 then went to use his bodyweight to keep the lift from falling. The lift fell and struck employee 2's leg causing a fracture. Employee 1 was in the lift basket when it impacted a staircase and he hit his head. He sustained several injuries including a head contusion, a broken pelvic bone, broken ribs, a spinal injury, and a lower extremity injury. Both employees were hospitalized.

At about 11:15 a.m. on February 21, 2022, an employee was loading a reflector onto a punch machine. The punch activated and cycled. It came down on the employee's left hand, resulting in partial amputations to the middle and index fingers to at least the first knuckle.
