Employee 1 was in the lift basket when it impacted a staircase and he hit his head. He sustained several injuries including a head contusion, a broken pelvic bone, broken ribs, a spinal injury, and a lower extremity injury. Both employees were hospitalized.

At about 11:15 a.m. on February 21, 2022, an employee was loading a reflector onto a punch machine. The punch activated and cycled. It came down on the employee's left hand, resulting in partial amputations to the middle and index fingers to at least the first knuckle.

An employee was operating a yard mule and fell from the vehicle, resulting in a broken hip.

At about 3:50 p.m., an employee was driving a forklift when it struck another, stopped forklift. She suffered an injury to her left ankle and was hospitalized.
