An employee was operating a meat bone saw and using a pusher when their hand slipped and struck the blade. Their right middle and index fingertips were amputated.
An employee was clearing a jam on a conveyor belt when the hand tool they were using to clear the jam was pulled into the machine and passed the metal guard. The employee's left middle finger contacted the fixed, grated guard, resulting in a partial amputation. The machine was energized at the time.
