An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was tightening a connection on an arsine cylinder within an arsine fill cabinet. The employee was exposed to arsine gas and started experiencing symptoms a few hours later. The employee was hospitalized for exposure to arsine gas.

An employee was aligning a box before palletizing. The machine loaded another pallet and trapped the employee at the knees, resulting in skinned/bruised shins.

An employee was troubleshooting a packaging machine when a case packer push cylinder retracted and amputated their fingertip.

On September 24, 2024, an employee was using a powered industrial truck (PIT) to pick orders. His right foot made contact with an unmanned PIT, resulting in severe lacerations to three toes on his right foot.

An employee was brazing when her foot got caught in a brazing line, resulting in her falling and hitting her left hip and knee. Approximately four hours later, she was at a different work station cleaning cooler doors. When she went to leave that area, she tripped over an airline and fell onto her left hip, knee, and elbow. Both of these incidents resulted in her falling onto the concrete floor on her left side. Later, when returning from her lunch break, she fell in front of her vehicle and landed on her left side. The employee suffered a fractured left femur and hip, resulting in hospitalization.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

Two employees were setting up a mandrel in the spindle of a machine. The mandrel was side-shifted while an employee's hand was on the mast of a powered industrial truck. The employee suffered crushing injuries that resulted in amputations to the middle and ring fingertips.

An employee had been loading a double deck press. The employee saw a mold had been sent in with the C-hook still attached on the belly bar. As he went to remove the hook from the mold, his right thumb got caught on the C-hook. The employee's thumb was partially amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.
