An employee was adjusting a mold during setup on the vibrating table. The mold fell and caught the employee's hand against the machine's worktable. The middle finger was amputated and two other fingers were severely lacerated.
An employee dropped his utility knife into a mixer machine. As the employee went to retrieve the knife, his right arm was struck by the mixer blade, resulting in a laceration that required hospitalization.
