On February 26, 2022, an employee parked a truck at a loading area and began using an electric stand-up pallet jack to unload it. As he backed out of the trailer, the dock plate popped up, causing him to fall to the ground. The rear of the pallet jack ran over both of his feet; the left foot was sprained and the right foot was broken.

On February 24, 2022, a truck driver was standing in his trailer, doing paperwork as part of a delivery. He fell out of the trailer to the ground 4 feet below and suffered a neck fracture.

On December 3, 2021, at the unloading dock, an employee was using a forklift to unload boxes from a trailer onto a pallet. As he was backing out of the truck's trailer and dock plate, the employee's left leg hit the dock's guardrails, resulting in a fractured fibula and tibia that required hospitalization.

On 10/09/2017, at 2:30 p.m., and employee was struck by a palletized load on a pallet jack, requiring hospitalization.

While standing on a loading dock to the side of a forklift, an employee was bumped by another forklift, fell down, and sustained a deep laceration to his left leg below the knee, requiring stitches.

An employee was walking on a job site when they were struck from behind by a pickup truck. The employee sustained fractures to the arms, pelvis, and legs.

An employee was leaf blowing to help clean the road. An excavator began backing up and caught the employee's foot, causing her to fall to the ground. The employee sustained the loss of a tooth, abrasions to the face and hand, and a cut to the foot.

An employee was tying a hose. A truck backed up, pinching the employee's leg between the hose and hammer union, resulting in a right fibula fracture.

A crew was setting up to clean out a squeegee machine. The controls of the machine were struck by a spray wand, shifting the machine from park to reverse. When the machine was started, it lunged backward and caught an employee s leg against a water truck. The employee's leg was fractured.

An employee was struck by an excavator. Their legs were caught and crushed under the tracks, leading to amputation of the legs.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

An employee was hooking up a set of 28-foot trailers, attaching a converter dolly to the lead trailer's pintle hook. He backed up the lead trailer and parked it a foot from the converter dolly, then lifted the converter dolly and rolled it to the lead trailer to set it onto the hook. The converter dolly kicked up, and the employee's left hand was caught between it and the trailer. He suffered a severe laceration to the palm between the ring finger and middle finger.

An employee was cleaning the windshield of his truck. When he stepped down from the truck, he lost balance and reached to catch himself using the side mirror. The impact fractured his left wrist. The employee was hospitalized and required surgery.

An employee was standing on the DOT bumper on the back of a standard 53-foot trailer. The employee lost his footing and his grip on the handle bar, and fell approximately 42 inches to the ground. The employee was hospitalized with a fractured left hip/femur.

A driver was making a delivery. As he was climbing onto his trailer to get boxes, the strap he was using to pull himself up broke. He fell from the back of the trailer to the ground, landing on his right leg and breaking his femur.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
