When the employee turned the conveyor back on, the tape was pulled back up and wrapped around the employee's left arm and pulled it into the conveyor, resulting in an upper arm fracture.

An employee was climbing a 6-foot A-frame ladder that was placed on a table to access and repair an oscillating fan mounted on the wall. The ladder was leaning against the wall and as the employee began to climb the ladder, the table moved away from the wall. The employee fell to the concrete floor and sustained bruised ribs and lacerations that required staples.
