On October 18, 2023, an employee was performing a die change on a hydroforming machine when one of the supports fell out, causing the die to fall and crush his left index finger against the machine. The employee suffered a partial amputation.
An employee was working on a 32" hydroform. The piston had been removed from the equipment. The collar on the center piston slide down and caught the employee's right hand injuring the index finger. It was later amputated at the hospital.
