Final narrative
The injured employee was in a trailer working to level a load that had shifted during transport. Another employee was using a forklift to assist. The load shifted and fell on the injured employee, causing torn ligaments in their knee and a fractured foot.
An employee was operating a lift truck through racking while wearing a face piece for warmth. Their face piece shifted and as the employee went to fix it, the vehicle struck a rack. The employee's left foot was pinned between the vehicle and the racking, resulting in a big toe fracture and lacerations under the big and second toes. The employee was hospitalized.
The injured employee was unloading a trailer with a forklift. He had just stopped the lift and stepped from the operator compartment onto the warehouse floor. Another forklift pulled forward pinning the injured employee between the two forklifts. The injured employee sustained multiple back and leg fractures.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
