An employee was operating a machine that presses sleeves into rod guides. The employee went to straighten a sleeve that was misaligned on one of the assemblies moving into the ram position. The employee s left middle finger got caught between the ram and the sleeve, resulting in a fracture and a fingertip amputation.
An employee was troubleshooting a stamper machine. The part-sensing proximity switch was malfunctioning. The employee reached through the part access opening in the guard to adjust the proximity switch bracket.
