An employee was monitoring the workstation for an extruder. The employee was working to unjam/pull wax material off the rollers when their gloved left hand was pulled into the rollers. The employee sustained amputations to their index finger, middle finger, and thumb. The machine was running at the time and was not guarded.
A maintenance technician was using a prybar to remove the last stuck bolt from a deflagration panel when a metal chute around the panel fell away, pinching his left forearm between the access panel opening of the chute and a railing.
