An employee was adjusting his knee brace while standing at the top of a flight of stairs. He lost his balance and fell down the stairs, striking his head and hand on the concrete floor. The employee sustained abrasions, a swollen eye, a broken finger, and lacerations to the head that required stitches above an eye.

An employee was patrolling a store using an electric patrol scooter when they lost their balance and sustained an ankle fracture.

An employee was operating a forklift to move panels around the dock area when the vehicle struck a joint where the two floors meet. The employee lurched forward and struck his head on the steering wheel. The employee then fell from the vehicle and sustained a dislocated disc in their back, requiring hospitalization.

The employee suffered a skull fracture, a fractured L1 vertebra, a broken coccyx, a laceration/abrasion to the head, and a brain bleed.

An employee was riding an ATV, catching up with a stray cow to return it to a pen. The ATV tire struck a hole or other obstacle, causing the employee to be ejected and fall to the ground. His leg was broken and he was hospitalized.

On November 14, 2023, an employee was standing at a loading dock, with one foot on the back of a truck and one foot on the lift gate, when the truck pulled away from the dock. The employee flew 6 feet and fell to the concrete floor, suffering injuries to the head, left eye (broken blood vessels), right thumb (dislocation), and back. The employee was hospitalized.

The employee was hospitalized due to a broken right hip; broken ribs number three, five, and seven on the right side; a chest contusion; a laceration to the top of the head; and a concussion.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
