An employee was on the second floor of a building kneeling over a void space. As they fired a powder actuator to shoot a rivet into a metal track, they lost balance and fell through the space, resulting in fractures to the ribs and left collar bone as well as brain hemorrhaging. The employee was hospitalized. Fall protection was not worn at the time.
