On October 14, 2019, an employee was placing a wire rope choker around a pedestal that was going to be moved by an all-terrain forklift when his hand was pinched between a piece of rigging (wire rope choker) and the load that was to be moved. He was hospitalized with fractures to three of his fingers and a laceration to the palm of his left hand.

An employee had been working outside in hot and humid conditions. The employee became dehydrated and was hospitalized for heat exposure.

An employee and a coworker were moving a headache rack when the coworker dropped his side. The employee's finger was then caught between the headache rack and the C channel, amputating the right ring fingertip.

An employee was setting up the table saw to perform dado cuts on a door. The material kicked back and the employee's right hand was pulled into the blade. The employee was hospitalized for surgery on his index finger, middle finger, and thumb.

An employee was fixing ceiling tiles while working from a ladder when they fell to the floor and sustained a hip fracture.

An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.

An employee was unloading appliances at a customer location for delivery. The employee was moving a water heater to be placed on the liftgate when he stepped backward and fell to the ground. The employee sustained rib and back injuries.

An employee was coming down a portable ladder when they slipped and fell to the ground, resulting in a right forearm fracture.

An employee had just driven a vehicle into a rail car and was going to start a new deck. He was moving a bridge plate fixture that enables them to drive from one rail car to another when the weight of the plate pulled him off the deck. He fell and struck his head on the ground. He also struck the piece that secures cars together and sustained a wound to a cheek, two back fractures, ruptures to the kidney and spleen, internal bleeding, and severe bruising to the right arm and side.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was installing insulation in a wall when she tripped on a hole (12" deep) and fell, hitting her head on the concrete floor. The employee sustained fractures to her leg and wrist, and lacerations to her head. The employee was hospitalized.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.

An employee was operating a power drill when it slipped. The drill bit punctured the employee's hand and they were hospitalized.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
