An employee was clearing a jam in a conveyor and manually moving packages when he lost his balance and fell head first onto the side of a chute. The employee suffered lacerations and contusions to the head above the eyebrow. The employee was hospitalized.
A ski Instructor was teaching a lesson to two students. The instructor skied downhill first, demonstrating the exercise. He then turned to watch the students and slid on an ice patch, caught an edge, and fell onto his right side/hip. The employee sustained a fracture to their right hip/upper leg.
