An employee was placing a part in an automatic setter when their middle finger was caught in the machine and amputated.

A maintenance employee was responding to an extruder machine that had a malfunctioning water cooling unit. When the employee went to see what was wrong, the machine released hot water and steam, which struck the employee in the arm, torso, and feet. The employee suffered burns to the skin and was hospitalized.

A maintenance employee was performing maintenance on a cutter machine, replacing the drive gear and motor belt. As the employee was adjusting the belt, the machine cycled and rotated the flywheel. Two bolts on the side of the flywheel caught the employee's right hand, crushing and lacerating the index fingertip and damaging the middle fingernail. The right index fingertip was surgically amputated just below the fingernail.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was preparing to cut rubber stock on a pneumatic cutting machine when their middle and ring fingertips were amputated by the cutting blade.

At about 12:25 a.m. on September 3, 2025, an employee was troubleshooting a malfunction involving a slitter whose arbor was not rotating as expected. The employee was observing the slitter's operations when the arbor completed its cycle and the employee's right index finger was drawn into the slitter's 4-inch, non-rotating cutting blade. The fingertip was amputated.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
