On January 12, 2024, an employee was moving metal rolling racks loaded with automotive mufflers when their left hand was pinched between two racks, resulting in an amputation of the ring fingertip before the first knuckle.

The employee was operating a forklift to unload a rack from a trailer at the dock. The employee stopped the forklift and dismounted it. The forklift rolled forward and the employee's right leg was pinned between the racks and the forklift causing a right ankle fracture.
An employee was giving directions to a forklift operator who was stationary by a dock door as another forklift backed out of a trailer from a different dock door. The second forklift turned sharply to the left and pinned the employee between the two forklifts.
