The parking brake on a forklift was not set and it began to roll backward. An employee was struck by the forklift while attempting to stop the forklift from rolling backward. The employee sustained a fractured left tibia.

An employee was closing a trailer door and fell off the back of the trailer onto the ground. The employee sustained a head laceration.

An employee was reaching for clothes to hang on a Z-rack when they tripped over the Z-rack and fell, resulting in a broken elbow.

On November 5, 2019, at 2:07 PM, an employee was walking between a tote container filled with empty hangers on the floor and a Z-rack. As she turned the corner, she tripped over the base of the Z-rack and her face struck the base of the Z-rack bar. She sustained a compound fracture to her left arm and a right wrist fracture, requiring hospitalization.

An employee attempted to crush a box under foot and fell to the floor, breaking the right leg. The employee was hospitalized and had surgery.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was taking off screws from a large picture frame using a battery-powered drill with a screwdriver bit. They suffered a finger amputation due to the picture hanging wire attached to the picture frame.

An employee was sweeping the parking lot when they stepped backward, tripped, and fell. The employee suffered a fractured left hip and was hospitalized.

An employee was entering a resident's room to administer medication when they slipped and fell. The employee was hospitalized with a fractured arm.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
