Two employees were installing a machine part (a compactor). The injured employee was holding the compactor as it was pushed into place, and their left middle finger was caught between the compactor and the rail it was to sit on. The fingertip was amputated. The incident took place during a training activity.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.
