Final narrative
On February 7, 2024, at approximately 5:00 p.m., an employee was attempting to clean a pneumatically operated vacuum ball valve when their left index finger became caught in the ball and valve, resulting in an amputation of the finger pad. The machine was not locked out at the time of the incident.
On April 21, 2025, an employee was working to change the rubber leveling feet on an 180-pound mixing silo scale. After the leveling feet were replaced, the employee was removing a wood cribbing block that was used to support the raised scale, when the scale slipped. The second finger on the employee's left hand was crushed between one of the leveling feet and the wood block, resulting in amputation.
