On May 21, 2024, an employee climbed a 28-foot extension ladder and unhooked the drop. This released tension that caused the ladder to shake and throw him off. He fell to the ground, suffering broken ribs and a ruptured spleen.

On November 21, 2019, an employee suffered a back strain while climbing a ladder.

An employee was working from a step ladder to install cable at a bank when the ladder slipped out from underneath him due to snow and ice on the walkway. He broke his leg and was admitted to the hospital.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee was standing on the surface of a steel beam about two feet wide. He slipped and fell backward, landing on the concrete ground about 20 feet below. The employee sustained a broken pelvis.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

An employee was working on a platform, raised 6 to 8 feet in the air, to get something off a rack. The employee fell from the platform to the ground and sustained a head injury and injury to multiple body parts.

An employee was preparing to unjam an auger shaft while standing on a forklift-elevated, job-made platform. The employee's wrench slipped off the equipment, causing them to lose their balance and fall. The employee landed on the ground 14 feet below and suffered fractures to the orbital bone, left elbow, and lower back.

An employee in an aerial bucket was attaching a cable to a pole when they contacted a power transmission fuse and sustained burns to their left hand, arm, and shoulder.

An employee was on a ladder that was leaned against a wire strap while installing cable service. The ladder slipped and the employee fell, resulting in a broken lower left leg.

An employee was working from the bucket of a bucket truck. After completing his work, the employee lowered the boom of the truck and exited the bucket onto the truck. As he was exiting, he tripped on the door frame opening, fell onto the rear deck of the truck, and then to the ground. The employee suffered an injury that required hospitalization.

An employee was standing on a ladder to perform repairs on a cable case when they fell approximately 6 feet to the ground, resulting in a left foot fracture and dislocation.

An employee was troubleshooting cable television services at a residential home. While ascending the extension ladder, the ladder slid, causing the employee to fall. They sustained four vertebrae fractures as well as bruises to the head, shoulder, and legs.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
