Final narrative
An employee was hooking up chains to blocks on the crane arm. The employee slipped and tried to catch themself when their right hand was caught in the crane wires that then began to spin around the pulley. The employee sustained amputation of the right middle finger and lacerations to the ring and little fingers.

A sanitation employee entered the chemical room and slipped on the wet floor. When he slipped, he kicked over a bucket containing cleaning chemicals. The chemicals splashed onto the employee's clothing and burned his skin.
