An employee was performing maintenance on a tray packer. They put a piece of cardboard down onto the belt of the tray packer and stood on the belt. The belt started, causing the employee to lose balance and fall approximately 2 feet. The employee was hospitalized with a fractured ankle and required surgery.

An employee was walking down a flight of stairs from a catwalk to a production area. The employee missed a step, fell down the stairs, and suffered broken ribs.

An employee was clearing a jam on a machine when their right index fingertip was amputated by the sealer. A guard was in place at the time.

On January 16, 2023, an employee was pushing a cr me tub under the vibratory system when her right-hand fingers were caught between the handle of the tub and the bottom of the vibratory system s framework. The employee sustained fingertip amputations to the third and fourth fingers on her right hand.

An employee was working to build an electrical substation. He was cleaning out the inside of a fixed section of a PVC pipe on the end that was furthest from an excavator. The excavator was outside of the trench and more than 25 feet away when it moved a pipe, pinching the employee's right arm between the two pipes. The employee's forearm was fractured.

An overhead crane lifted the equipment, causing the employee to be pulled by his harness. The employee was pulled off the walkway and swung over the wall of the kiln. The employee also sustained multiple fractured ribs and fractured vertebrae.

An employee was in a scissor lift, preparing to strip formwork from the underside of a concrete deck. As the lift was ascending, the employee's left little finger was pinched between the lift rail and the concrete beam above. The employee was hospitalized, and the soft tissue of their left little fingertip was amputated; the finger's bony tuft was also damaged.

An employee was conducting maintenance work on an stainless steel auger feeder (flexible screw conveyor) in the seasoning room. The lower end of the feeder came loose from the hopper. The employee lifted the auger to position it when the auger feed initiated and energized the auger. His hands were pulled into the auger, resulting in hospitalization with a partial amputation of the right thumb.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
