Federal OSHA recorded a severe workplace injury at Industrial Apparatus Service, 1320 Jim Gilligan Way, BEAUMONT, TEXAS 77705 on — Fractures , affecting the Trunk and other upper extremities.

An employee was standing on a ladder next to a motor to obtain a measurement reading from a dial indicator. The employee leaned over the motor and inadvertently leaned onto a overhead crane remote, causing the motor to fall toward the employee. The employee jumped from the ladder to prevent being struck by the motor and fell 10 feet to the concrete floor, resulting in a right rib fracture, a right scapula fracture, and contusions.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below.
