An employee was cleaning saw dust off of a straight line rip saw after turning it off when his fingers contacted the blade that was still spinning, resulting in the amputations of the left middle and index fingertips to the first knuckle.

Spartan Education Group LLC

An employee was performing a magneto timing service on a four-seat, single-engine, fixed-wing aircraft. The timing service requires the propeller and the engine to be moved/rotated. When the propeller rotated, it struck the employee, causing a head injury. The employee was hospitalized.

Raceland Raw Sugar, LLC

The injured employee and a co-worker were moving a tank car loaded with molasses from the loading station to the designated side of the facility using a wheel loader to pull the rail car. The employees reached an incline and applied the handbrake. While they repositioned the wheel loader to push the rail car the remaining distance to their destination, the rail car started to move backward. The injured employee climbed onto the rear platform of the car to tighten the brake, but the rail car collided with the loader bucket and the employee suffered fractures to their right femur and tibia.

Dairyland Power Cooperative

An employee was adjusting the turnbuckle on the sash chain of a rolling overhead fire door using pliers. The turnbuckle detached from the bracket and the bracket rotated, striking the employee's forearm. The employee was hospitalized with a laceration.

ISCO INDUSTRIES, INC.

An employee was cutting a 7-foot pipe using a band saw. He went to wipe moisture off the pipe with a rag and the rag became entangled in the moving saw blade, pulling his right hand into the blade. The employee sustained a partial amputation to the fourth distal phalanx and third digit.

Clarion Bathware

An employee was working in the grinding room on a production line for bathtubs. The tubs are laid on a conveyor and have wooden blocks for feet. The conveyor moves the tubs onto a saw that cuts off the wooden blocks. The saws are protected by expandable metal guards. The conveyor then moves the tubs through an area lined with coarse horsehair brushes before reaching the discharge area, where the employee was working. The employee was drilling a hole behind the metal guarding when their right hand went into the horsehair brush area and contacted a saw blade, resulting in a laceration on the back of their right hand that required surgery.

An employee was taking a part out of a press. The press's injector pin came down on his right big toe, crushing it. He was hospitalized.

An employee was fusing two HDPE pipes in a machine when the machine amputated his left index and middle fingertips.

Lippert Components, Inc.

An employee was operating a stand-up forklift to close doors around the facility. As the vehicle was traveling in reverse, the forklift struck a wall and the employee's left foot was pinned between the forklift and the wall. All of the toes on the employee's left foot were amputated.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

McAneny Brothers, Inc.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
