An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was diagnosing the lack of flow of product to a powder bin. The employee removed the rotary star valve below the bin. While he was reinstalling the valve, his right middle finger was crushed between its shaft and its housing. The finger was partially amputated.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was conducting routine maintenance on a mixer. New lid pins had been installed, and the employee was rotating by hand to ensure they had been installed correctly. The employee's left middle finger was caught between the lid and top of a pin. The momentum from the mixer continued, causing a crushing injury to the finger. The employee underwent a medical amputation from the top knuckle to the tip of the finger.

An employee was using a hand truck to load a refrigerator onto a box truck. The refrigerator was set down near the back of the truck and the wheel of the hand truck became stuck in the box truck. When the wheel dislodged, the hand truck and the refrigerator fell on the employee's left arm resulting in injury from the hand to the elbow, including tendon damage.

An employee was closing the liftgate to a box truck when their finger was caught in the clamp/gear as it was folding up. The employee pulled his finger out and the left index fingertip was amputated.

An employee was moving an appliance with a hand truck, slipped on water that another appliance had leaked, and fell. The hand truck and appliance fell on him. The employee suffered multiple fingertip amputations as well as back and abdominal injuries.

An employee was manipulating a 3-foot tall industrial warehouse rolling gate to determine why it was squeaking during operation. The gate fell over and struck the employee's left leg, causing him to fall off a loading dock and hit his head on the ground. The employee sustained brain injuries, three cranial fractures, a left leg fracture, and rib fractures.

An employee was moving a refrigerator with a two-wheel dolly when he tripped and went backward. The appliance fell on his leg and broke his thigh bone. He was hospitalized for surgery.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
