A diver was underwater cutting steel floor beams from an anchor span truss when the anchor span moved and pinched his hand between the previously cut lateral beam and the truss. The employee was trapped for a few minutes until the steel structures moved and released him, allowing him to surface. He suffered a right ring finger and pinky fingertip amputation, as well as a right middle finger laceration.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

An employee was disassembling a pressurized unit using a driver drill. When the unit was disassembled, it caused a kickback from the tube head, causing the drill to strike the employee's left forearm. They sustained a diaphyseal fracture to their left radius and ulna, which required surgery.

An employee was using a box cutter when it cut the tip of his left thumb resulting in a fingertip amputation.

While working at a customer's residence, an employee was cutting tape with a box cutter to wrap insulation for a piece of pipe. The box cutter struck a hardened object within the wall causing it to bounce back and puncture the employee's right eye. The employee was hospitalized.

An employee was using the pipe-and-wrench method to loosen a seized bolt on a roof truss. A technician from another company was on the other side of the truss doing the same action with the same method. The employee's socket slipped off the bolt and his left hand was crushed against the truss, resulting in injuries to the index and little fingers, requiring hospitalization, surgery, and amputation of the index finger.

An employee was working to assess the operation of a high-pressure water nozzle. The water system was activated and the employee was cut by high pressure water on his left hand where the palm meets the wrist. The employee was hospitalized.

An employee was performing torching work when metal fell on top of their foot and fractured the smallest toe.

An employee was working to push items in a trailer dumpster with his feet; his back was against a building wall. He fell to the concrete ground, resulting in fractures.

An employee was guiding a high-pressure water hose through a 4-inch gap on a horizontal condensing tube when the hose blew apart. The hose left the opening and the fitting at the end of it struck the employee on the side of his face. The employee sustained blunt force trauma to the left side of his head, including a laceration from his left temple through his ear.

On July 24, 2025, an employee was shoveling material when a piece of steel slag fell from a nearby furnace and struck them in the back. The employee was hospitalized with fractures to their thoracic vertebrae and a metatarsal.

An employee was closing a gun locker in his automobile trunk when his left middle finger was partially amputated by the lid.

An employee was ascending a flight of stairs after exiting their vehicle in an adjacent employee parking lot. The employee stepped to the right to avoid another employee on the left who was descending the stairs. The employee lost their balance and fell on the stairs. They were hospitalized with a laceration/abrasion to their chin, a fractured left humerus, and bruising to their left knee.

A mail handler was moving BMCs on the loading dock when they fell from the loading dock to the concrete ground below. The employee was hospitalized with head trauma.

An employee was on an A-frame ladder while preforming demolition of a ceiling. A section of HVAC ductwork fell on the employee and knocked them to the ground approximately 12 feet below. The employee was hospitalized with a concussion and lacerations.

On the morning of July 24, 2025, an employee was conducting synthesis of a highly toxic experimental chemical (nerve agent) in a chemical fume hood. The operation was completed at approximately 11:30 AM. The employee was exposed to the chemical and began experiencing symptoms of illness/poisoning around 1:30 PM and was hospitalized.
