An employee was operating a riding pallet jack and making a turn toward a dock door when the pallet jack slid and struck a parked pallet jack, catching his foot between the two pallet jacks. The employee was hospitalized with a right foot fracture and required surgery.

An employee was moving pallet jack out of a charging station in order to put another jack in the charging station and struck metal racking. His right foot was pinned against the metal racking, causing a heel laceration.

An employee was operating an electric pallet jack when he was pinned between the jack and a rack. He suffered a leg injury.

An employee was getting a pallet with a pallet jack. His pallet jack moved and pinned his left leg against another pallet jack, breaking the ankle.

An employee was shutting down a restaurant's alarm when his left pointer finger was caught between the door jamb and the door, amputating his left pointer fingertip without bone loss.

An employee was rolling up the landing gear on his trailer when the handle began to unwind and struck his face, fracturing his jaw. The employee was hospitalized and required surgery.

A maintenance employee was working to place a 120-pound metal screen on top of a machine before installation, with the help of another employee. The injured employee's right ring finger was pinched between the screen and the machine and the fingertip was partially amputated.

An employee was assisting three coworkers with loading a fall protection metal storage cage onto a wooden pallet. Their right index finger was pinched between the pallet and the bottom of the cage, resulting in a fingertip amputation.

An employee was helping another employee load stock material onto a press brake when the stock slipped. The injured employee's right index finger was caught between the press brake die and the stock, resulting in amputation at the first knuckle.

An employee was moving a toolbox from a pallet to the sales floor when his left middle finger got caught between the toolbox and the concrete floor, resulting in a fingertip amputation of the distal digit.

On October 16, 2025, an employee operating an order selector was passing a moving scrubber when the scrubber suddenly stopped. The order selector struck the back of the scrubber, resulting in a cut to the employee's left ankle.

On September 5, 2025, an employee was walking in the parking lot to get product off the dock. A 26-foot box truck was backing up and pinned the employee against the dock plate. The employee sustained fractures to three vertebrae in their back.

On August 30, 2025, an employee was operating an electric double-pallet jack. He was coming out of the new extension through the separation curtains when his pallet jack struck an electric forklift. The forklift landed on the employee's right foot, fracturing the big toe and second toe.

An employee was changing brake pads underneath a trailer unit. They used a hydraulic floor jack under an axle to secure the trailer in an elevated position. The trailer rolled and collapsed while the employee's head was underneath the air bag plate, resulting in a concussion, a laceration to the left side of their face, and a bruise on the right side of their face. The employee was hospitalized.

An employee was pushing an empty onion cart when his left little finger was crushed between the cart and a meat rack. The finger was partially amputated.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
