An employee was exiting the cab of a semi-truck when their foot slipped and they fell to the asphalt below, resulting in two pelvic fractures.
An employee was changing a tire on a tractor. The jack stand that the tractor was on had a piece of wood underneath because the ground was not level. The piece of wood cracked, and the jack stand slipped. The tire came off the lugs and struck the employee. The employee was hospitalized with a ruptured spleen and had surgery.
