An employee developed lower back pain after climbing up fixed ladders to reach a roof, then beginning to route wire into an air handling unit. When the employee knelt to pick up rope, the back pain became severe. The employee was hospitalized, having suffered a sprain to a back muscle.

An new employee was participating in an on-the-job training. The employee was demonstrating a defensive knee strike tactic when he suffered a left ankle injury. As he was pivoting to sprint backward as part of the simulated disengagement drill, his left ankle rolled and he sustained a torn Achilles tendon that required surgery.

An employee was conducting field operations training outdoors. Training consisted of walking, standing up, and being in prone positions. The employee was hospitalized with rhabdomyolysis.

The employee stepped to the right to avoid another employee on the left who was descending the stairs. The employee lost their balance and fell on the stairs. They were hospitalized with a laceration/abrasion to their chin, a fractured left humerus, and bruising to their left knee.

An employee was working on an electrical panel in a mechanical room when an arc flash occurred. The employee suffered burns.

At about 3:10 p.m. on October 3, 2025, an employee was inspecting a car. Two dogs that had been in another car were leashed and tethered to a bollard. As the employee inspected the first car along with a narcotics detection dog, one of the other dogs came loose and attacked the narcotics dog. The employee was separating the dogs when the other dog bit his left ring finger. The last joint of the finger was injured and part of it was bitten off.

An employee was climbing a fixed ladder to perform maintenance in an attic. While unlocking the attic entry, the employee fell from the ladder more than 10 feet to the concrete below. The employee suffered a fractured hip, pelvis, tail bone, arm, and elbow, resulting in hospitalization and surgery.

A building engineer was inspecting a generator. As he closed the generator enclosure door, his right ring finger was caught between the door and the door frame. The fingertip was amputated.

An employee was walking back to a carrier case with mail in her hand when she tripped over a tub. Her back overarched as she fell, resulting in a fractured back that required hospitalization.

An employee was driving a utility tractor rig when it struck the rear trailer of another utility tractor rig, which was traveling in the opposite direction. The glancing impact caused damage to the cab, and he sustained a laceration on right side of his face, a cervical fracture, two fractured left ribs, and a pelvic fracture. The employee was hospitalized.

An employee was washing an aircraft from a scissor lift when they fell to the floor. The employee sustained a back injury and lacerations to their head and neck.

An employee was working with a drill and a metal cable. The drill caught the cable and caused an amputation to the employee's left ring finger.
