Final narrative
An employee was operating a forklift and dumping scrap metal into a recycling bin. He was picking up metal scraps when a metal pallet slid off the forks of the forklift and caught his right little finger against the scrap bin. The employee's finger was partially amputated.
An employee had staged a forklift and was preparing to release a latch from the attached bin. The latch did not allow the bin to tilt forward. The employee manually pushed the bin while holding the latch open. When the bin bounced back, the employee's left middle finger was crushed, resulting in partial amputation via surgery.
