On May 27, 2025, at approximately 12:30 a.m., an employee was operating a forklift and taking down merchandise. The forklift struck a rack and the employee sustained a fractured right arm. She was hospitalized.

An employee was cutting bone-in short loin on a band saw when he sustained a soft tissue injury and was hospitalized.

An employee was climbing a 10-foot fixed ladder while carrying a small box in one hand. The employee fell from the seventh step to the floor of the back room and sustained a right ankle fracture.

An employee was closing the door of a safe when his finger was pinched between the door and the frame. The employee sustained a left middle fingertip amputation.

An employee was retrieving supplies from a cabinet while standing on a step stool. The ring on their middle finger was caught on the shelving as the employee descended the step stool, resulting in a finger avulsion.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

Employee 1 was investigating a gas odor. While employee 1 was checking the equipment, a gas explosion occurred, resulting in a fire that burned the employee's face. Employee 2 entered the area and a second explosion occurred, causing burns to their face and arms. Employee 2 was hospitalized.

An employee tripped while walking from one store register station to another. Her forehead struck a register, and she suffered fractures to two vertebrae in her neck. She was hospitalized.

An employee was pulling a display shipper using the provided strap. The strap broke and the employee fell to the floor, suffering a broken wrist.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
