An employee was conducting maintenance activities on solids control equipment when his right hand was caught in the equipment's grinder, resulting in the amputation of his right-hand fingers.

An employee was adding production material to a machine by hand and watching the canning process. One of the products was loaded upside down causing it to detonate. This set off a chain reaction of nearby products, causing a flash fire. The employee suffered severe burns across 80% of his back, 60% of his chest, and 90% of his left arm. He was hospitalized.

Three employees were plugging an oil and gas well when one of the onsite explosives detonated prematurely, injuring one employee. The employee sustained multiple lacerations and fractures to the waist and legs.

Employees were setting up for an event to showcase fireworks for a municipality. The injured employee was setting up the tubing for the mortar shells when a mortar shell ignited. An employee sustained burns to their face, arms, and abdomen.

An employee was preparing pyrotechnic materials utilizing screen/sieve equipment that is operated in a unoccupied bay that is interlocked. After sieving/screening was completed, the employee entered the bay to manually transfer the pyrotechnic material and noticed materials were stuck to the sieve/screen. The materials detonated during the transfer. The employee sustained first-degree burns to the face and third-degree burns to the hands.

An employee was installing articles of pyrotechnics when pyrotechnics misfired and blasted, resulting in injury to their hands.

An employee was backing up a garbage truck at a landfill to empty the truck. A concrete platform gave way, causing the vehicle to slide down into the landfill pit 20 feet below. The employee was flung backward and forward, hitting his head on the dashboard of the vehicle. The employee was hospitalized with a concussion.

An employee was kneeling beneath a loading dock, inspecting the area for future installation of an automatic dock plate. The existing manual dock plate fell off the dock and onto the employee, fracturing their hip.

On June 24, 2025, at 3:25 p.m., an employee had just finished dumping his truck at the recycling facility when they took a break in their truck. They began to experience signs of heat-related illness and left the cab of the truck before temporarily losing consciousness and collapsing. The employee was hospitalized due to dehydration, heat exhaustion, and acute kidney distress.

An employee was descending a ladder. As he stepped down, his foot landed on a pipe, which caused him to lose traction and fall. He sustained multiple torn ligaments, a shattered tibia, and a compound fracture to the heel bone. The employee was hospitalized.

An employee was unloading a truckload of sulfuric acid/soda ash water mixture at a water treatment plant. Residual mixture in a hose cap spilled onto his right forearm, burning it. He was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A security employee was holstering her firearm when the gun discharged a bullet into her right leg. The employee was hospitalized.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.
