An employee was assisting a transport vehicle that was leaving the jobsite by watching for oncoming traffic. After the transport vehicle (18-wheeler) departed, a pickup truck pulled onto the street. The employee then turned toward the rear of the pickup truck as it was moving away from him and started running after it. The employee went to jump onto the bumper and struck his left leg on the ball hitch. The employee's leg was fractured.

The injured employee was securing a dolly to the back of a shop switcher (vehicle). The employee was closing the pintle hook on the dolly when the vehicle began driving. The injured employee was caught in the pintle, resulting in an amputation to their thumb without bone loss.

An employee was on industrial steel shelving conducting inventory. A forklift struck a beam of a shelf, knocked the shelf out of position, and caused the employee to fall 15 feet to the surface below. The employee sustained a concussion and a laceration to the left side of the head, fractured ribs, and a contusion to the left hip.

An employee was cutting a board using a circular saw. The saw pinched, kicked back, and cut the employee's left ring and little fingers. The employee suffered an amputation to the little finger.

An employee was using an aluminum extension ladder to remove gutters at a height of approximately 20 feet. As they were removing the gutters, part of the gutter collapsed and struck the ladder, causing it to move sideways. The employee fell from the ladder and sustained fractures to ribs on the left side and their right forearm.
