An employee was unloading scrap metal from a truck at a third party site. A metal rack was being pushed out of the truck when the employee's pants got caught on the rack, pulling them to the ground. The employee sustained fractures to the left side of their face, left scapula, and left wrist.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.
