A crew was clearing a clog in a sewer line in a residential neighborhood. An employee was using an aluminum pole to guide the hose down the manhole entrance. The pole contacted overhead powerlines and shocked the employee. The employee was hospitalized and several toes were medically amputated.

Inframark, LLC

An employee was setting a rebuilt water pump using a forklift and a chain after inspecting the area where the pump was going to be placed. While the employee's right hand was on a nearby valve flange, the water pump shifted while in the air and struck the employee's right index finger, fracturing it. The employee was hospitalized and required surgery.

An employee was cleaning a laptop charging cable when lightning struck. The employee began experiencing heart palpitations and hearing loss, resulting in hospitalization.

A Smyrna Ready Mix delivery driver was delivering concrete to a job site. He backed his truck up to a pump truck, then began preparing it to transfer the concrete. The pump truck's boom moved and came into contact with a power line, which broke and fell onto the employee's delivery truck. The employee was knocked to the ground and suffered electrical burns to both arms and both feet.

An employee was clearing a right-of-way for utility lines when a small sapling was cut and fell, causing vines to pull a tree down and into power lines. The vines also became electrified and were contacting the employee's feet, shocking them. The employee became ill and experienced slurred speech and convulsions, resulting in hospitalization.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.
