An employee sustained the amputation of their little finger in the chain drive of the planer mill.

An employee was working on the production line to flip cants. Cants are large square blocks of wood that can vary in length and width and are produced at the beginning of the cutting process when a log goes through the mill before being cut to specific lumber dimensions. The employee was flipping the wood so that the live edge is facing up when it goes into the next phase of cutting. This is done by manually rotating the block of wood on the conveyor. The employee s left hand was pinched between the wood and the metal portion of the conveyor resulting in a finger amputation.

An employee was cutting a board using a circular saw. The saw pinched, kicked back, and cut the employee's left ring and little fingers. The employee suffered an amputation to the little finger.

An employee was cutting wood with a circular saw. The saw kicked back, and its blade cut three fingers on the employee's right hand, resulting in an amputation.

An employee was repairing a photo eye on the infeed roll case when they dropped a tool. The employee went to grab the tool and his fingers contacted the chain and sprocket of the infeed roll case. His fingers were pulled into the pinch point and the fingertips were crushed, resulting in amputation of one fingertip.

An employee was troubleshooting dispersing equipment. A belt and pulley system was energized, and the moving belt caught the employee's right ring finger. He suffered a partial amputation at the first knuckle, below the nail.

An employee was cleaning the photo eye sensors of a piece of equipment between the saw that cuts lumber into lengths and boards. The employee stepped on a metal grate that gave way and fell 8-10 feet. The employee was able to remove himself from the conveyor. His tibia was fractured just below the knee.

An employee was working to get a log back onto a carriage. The employee came into contact with the running saw, which amputated their arm.

While refilling a lubricant tank, an employee observed that a pump was not operating correctly. While troubleshooting the issue, the employee reached toward the back of the pump to check for proper airflow. Their left index finger contacted moving components in the pump's piston mechanism, resulting in partial amputation of the fingertip.

An employee slipped while climbing a ladder on a skidder, fell to the ground, and suffered a tibia/fibula fracture. The employee was hospitalized.

An employee was lowering the bucket of a track loader that contained a grade stake and the grade stake punctured his foot, resulting in hospitalization and surgery.

An employee was using a pike to move a pallet of sugar that was stuck in the powder racking. The employee fell backward, landing on his right leg. The employee's leg was fractured and he was hospitalized.

An employee was working to fix the forks on a forklift when the fork(s) fell on her toes. The second and third toes on her right foot were fractured, lacerated, and required stitches.

An employee was delivering a 65-foot boom lift to a rental customer's construction site. The employee set up the ramp truck in the parking lot adjacent to the construction site. He elevated the boom 4-5 feet to clear a temporary fence. While driving the boom lift off the ramp, the wheels slid to the driver's side of the ramp truck, ultimately sliding off the ramp. The boom lift fell off the side of the truck and rolled onto its side. The employee sustained a fractured clavicle, a dislocated shoulder, separated/fractured ribs, a punctured lung, and a fractured left femur. The employee required surgery.
