An employee was unloading a rack of intravenous (IV) pumps from a truck when two of the four wheels became stuck on the lip of the truck ramp. Two other employees came to assist. One employee went to raise the ramp while the other two lifted the rack to free the wheels. The rack became free and began to go down the ramp and into the loading dock. While the rack was moving down the ramp, it turned sideways and flipped over, landing on the injured employee. The injured employee was hospitalized with a fractured T12 vertebra.

An employee was carrying lab supplies through a lab. The employee tripped over a table leg and fell, suffering a hip injury.

An employee slipped on ice and fell while walking on the loading dock, breaking the left hip.

An employee was working at her desk. She went to stand up and fell to the floor beside the desk. The employee sustained a hip displacement and required surgery.

A doctor was sitting on a rolling stool when it moved out from under him. He fell, landed on his left hip, and suffered a broken left femur. He was hospitalized, requiring surgery.

An employee went to sit on a chair when it rolled out from under her, causing her to fall to the floor and strike her head. The employee was hospitalized with a closed fracture to a thoracic vertebra.

An employee was validating receipts at the drive thru at the bank. While reaching for the validator, the employee slipped out of her chair, striking the back of her head on the concrete floor. The employee sustained trauma to her head.

An employee had been sitting at a desk. While standing up from the desk, her feet became caught in a wire that was under the desk, causing her to fall out of her chair. The employee sustained a fractured pelvis and was hospitalized.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee slipped on water in a hospital hallway, fell, and landed on his left hip. He suffered a contusion and fracture to the hip.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
