An employee was on a residential front-load refuse truck in the stand-up position on the passenger side. The top of the truck struck a tree that was hanging over the road and the employee was ejected. The employee was hospitalized with a fracture to his lower right ankle and required surgery.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
