An employee was performing rear brake shoe service on a truck. The employee was using a prybar to remove a spring when the spring slipped from the S-cam and amputated his left index fingertip.

An employee was cleaning a machine and bleeding its lines. After the gauge read 0, the employee removed the metal lid covering a filter. The lid popped off and struck the employee in the face, causing a broken nose, a broken jaw, and lacerations.

Employees were changing out a level switch on a vessel when the switch flew off under pressure and struck an employee's face. The employee sustained fractures and lacerations to their face.

An employee was guiding a high-pressure water hose through a 4-inch gap on a horizontal condensing tube when the hose blew apart. The hose left the opening and the fitting at the end of it struck the employee on the side of his face. The employee sustained blunt force trauma to the left side of his head, including a laceration from his left temple through his ear.

On 6/11/2025, a lineman was removing a dead tree off of a powerline that had fallen due to thunderstorms. The employee cut the top of the tree to lessen the weight of the tree. He then hooked a jib line to the tree to lift it off the power line and place it on the ground. As he was moving his bucket into position, the tree shifted and fell, and the powerline released its tension. This caused a whipping effect and either the power line or the jib rope struck the employee's jaw. The employee was hospitalized with two fractures to his lower jaw.

An employee was working to remove scrap from the vicinity of a scrap shaker tray located at the rear of a press when she suffered a laceration and partial amputation of her right index finger.

An employee was loading a die plate onto a rack when the plate tipped down and pinched his finger, lacerating it. The employee sustained an amputation.

An employee was placing a flattener die on a turret press. The die broke loose and fell into the turret. The employee's right hand was pinched between the turret and the die, resulting in a partial amputation of the little finger.

An employee was un-banding metal blanks from pallets. He stepped on a pallet where the boards of the pallet overhung the runners, causing him to lose his footing and fall to the floor. The employee sustained injury to his left hip requiring hospitalization.

An employee was clearing a jam in the spot welder when the machine activated and crushed their right thumb, resulting in amputation of the thumb tip.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
