An employee was exiting the building following the completion of their shift. The employee slipped and fell on a sidewalk that was covered with snow and ice. The employee sustained three fractures to their right ankle. The employee was hospitalized and required surgery.

On January 8, 2024, an employee tripped on the curb as he was walking onto the sidewalk, resulting in a hairline fracture to his left hip.

A security officer was following a suspected shoplifter who got into their SUV. The employee jumped onto the side footrail of the SUV when the suspect began to drive away. The employee stayed on the vehicle for several blocks until he jumped off. When the employee jumped off, he landed on his feet then fell to the ground hitting his head on the ground. The employee sustained dislocated left index finger, subdural hematoma/head injury, tibia/fibula fractures to the right leg which required surgical repair.

On 7/05/2025, an employee was involved in a verbal confrontation with a customer at the check-out area. Later, in the parking lot, the customer drove her vehicle toward the employee, hitting his leg. The employee sustained a fractured leg.

An employee was walking in the parking lot while on a break when they were struck by a vehicle, resulting in a head and leg injury.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
