An employee was removing wood scraps from a concrete pad where he was to deliver sawdust. The wood scraps moved and he fell to the concrete pad. The employee sustained a fractured greater trochanter on their left femur.

An employee was stepping onto a short ladder to reach and connect an electrical line when he slipped. The lower portion of the right side of his ribcage struck the deck at the top of the ladder, resulting in a lacerated spleen. The employee required surgery.

An employee was separating board lumber by size at the flat deck. Their left ring finger came into contact with a 22-inch spinning saw blade and was amputated.

An employee was walking across a trimmer saw when they lost balance and fell. The employee extended their right hand to steady themself and their glove was caught by the drive belt of a pulley. Their hand was drawn into the pulley, resulting in amputations to the index, middle, and ring fingers.

An employee was repairing a photo eye on the infeed roll case when they dropped a tool. The employee went to grab the tool and his fingers contacted the chain and sprocket of the infeed roll case. His fingers were pulled into the pinch point and the fingertips were crushed, resulting in amputation of one fingertip.

An employee was troubleshooting dispersing equipment. A belt and pulley system was energized, and the moving belt caught the employee's right ring finger. He suffered a partial amputation at the first knuckle, below the nail.
