An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

Kisatchie Midnight Express

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Lockheed Martin Mission Systems and Training

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

Allen Tharp LLC.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

Allied Waste Systems Inc

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

Staff Solutions LLC

An employee was cutting a ductwork hanger. The employee finished cutting one end of the duct and came down from the ladder. The employee was walking underneath the duct when the ductwork fell on them. The employee sustained a broken rib and required surgery for a vertebra.

A temporary employee was in the locker room when they fell and sustained a hip fracture.

An employee was holding the door for contractors who were leaving the property when his little finger was caught in the door. Half an inch of the employee's left little finger was amputated.

An employee was fixing an issue with seed cleaning equipment when he slipped on the ladder he was on. He went to catch himself and his hand went into a moving belt and pulley on the cleaner. The employee sustained the amputation of his right middle and index fingertips just below the fingernails.

An employee was riding an all-terrain vehicle (ATV) to move a bison herd. He was riding up the side of a steep hill when the ATV struck a rock and rolled, ejecting the employee before rolling over him. The employee suffered broken vertebrae in his back that required hospitalization.

An employee was clearing out dirt from an underground pipe when he was bitten by a rattlesnake on his hand.

An employee was climbing in the spreader area to remove a pasta jam when his right little finger was cut by the rotary cut-off knife. The employee sustained a laceration and fracture, resulting in an amputation.

USDA Forest Service Ranger Station

A smoke jumper parachuted out of a helicopter during a forest fire response, landed on steep rugged terrain, and suffered femur and rib fractures and a liver laceration. The employee was hospitalized.
