An employee was walking through a parking garage. She tripped over a curb and fell forward onto her right knee, then her left knee, both hands, and her face. She suffered an injury to the left knee that required surgery, as well as scrapes on the chin and both palms and soreness in the left wrist.

An employee had been loading packages into a vehicle. While exiting the vehicle, he tripped and fell, fracturing both ankles.

She reached in to stop the vehicle but the transmission shifted into reverse. The van turned and backed across a residential street, dragging the employee approximately 15 feet until it went up on a curb and the employee s left leg was pulled under the left front tire. She was hospitalized with fractures to her right wrist, right hand, and left fibula.
