Federal OSHA recorded a severe workplace injury at Appalachian Power Company North Charleston Service Center, 1107 Canterbury Drive, ELKVIEW, WEST VIRGINIA 25071 on — Fractures , affecting the Multiple trunk locations.

An employee and two crewmembers were dispatched to repair downed utility conductors after a tree fell onto the facilities and caused a power outage. The injured employee climbed the utility pole to untie the conductor so the team could pull the conductor up into position from the ground. The employee attached a web hoist to the conductor and needed to reposition the hoist to continue removing slack. As he resumed pulling slack with the web hoist, the pole made a cracking sound and subsequently fell in the direction he was working. The pole then fell to the ground with the injured employee still attached, resulting in fractured ribs, a compression fracture of their vertebrae, and a laceration to the chin.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was climbing down a ladder when it slipped. The employee fell, suffering a broken scapula and broken ribs. The employee was hospitalized.

An employee climbed an aluminum extension ladder inside a grain bin to help remove a stirring machine. As the farmer began to lower the stirring machine from the top, one of the ropes broke, causing the stirring machine to strike the ladder and knock it out from under the employee. The employee fell to the base of the grain bin and sustained fractures requiring hospitalization.

An employee was working on a 6-foot ladder that was leaned against a gas compression engine. The ladder feet slipped on the working surface, causing the ladder to become unstable. The employee fell off the ladder and suffered bilateral wrist fractures.

An employee was pulling a data cable through a ceiling. He fell through the ceiling, landing at ground level. He suffered a laceration to the forehead, a dislocated left little finger, a broken left ring finger, and a laceration to the liver.

The employee's left middle finger was caught between the lid and top of a pin. The momentum from the mixer continued, causing a crushing injury to the finger. The employee underwent a medical amputation from the top knuckle to the tip of the finger.
