An employee was reversing a stand-up forklift to transport a load outside the building when the forklift struck a table. The employee struck their left hand and left ankle against the table, and their left foot was pinched between the forklift and the table. The employee sustained fractures to their heel and multiple metatarsals.

An employee was lifting a vehicle leaf spring to position it on the press table. A bolt to hold it in position did not line up correctly and as the employee attempted to adjust it, the leaf spring fell on both of the employee's hands resulting in amputation of both index fingertips.

An employee was mounting a leaf spring onto a tooling machine when the spring moved. The tension in the spring pulled the employee's left index and middle finger against the backstop of the tooling machine, resulting in amputation of the left index and middle fingers.

Abbott Electric Inc

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

Cupertino Electric, Inc.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

The forklift drove into a rack in a warehouse, and the employee's right hand was caught between the rack and an upright supporting the forklift's overhead guard. The employee suffered a fracture and laceration to the hand, just below the little finger.

The employee was holding the bushing with their right hand and engaging the hydraulic press when their glove was pinched between the bottom of the bushing and the metal. The glove was drawn into the eye, and the employee's right index fingertip was pinched and amputated at the first knuckle.

An employee was inspecting leaf springs. They were trying to straighten a spring plate on a vise when their finger was pinched between the vise and the workbench, resulting in an amputation.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
